What Is Eyelid Surgery?
Eyelid surgery (technically called blepharoplasty) is a cosmetic procedure to improve the appearance of the eyelids. This can be accomplished by removing fat, usually along with excess skin and loose muscle, from the upper and lower eyelids. Blepharoplasty can correct drooping upper lids and puffy bags below your eyes—features that make you look older and more tired than you feel and may even interfere with your vision.

Eyelid surgery can restore a youthful lift to the upper eyelids by removing excess skin and tissue. What Are My Eyelid Surgery Options?
Upper Eyelid Surgery

Lower Eyelid Surgery
Puffiness under the eyes is not always due to a lack of sleep; for most blepharoplasty patients, bags underneath the eyes are actually small pockets of fat that have become more prominent due to muscle laxity. Some patients also develop ‘tear troughs,’ depressions under the puffy lower eyelids that result from age-related descent of upper cheek fat. Lower eyelid surgery allows Dr. Lattyak to remove or reposition the fat deposits, tighten loose muscles, and remove excess skin.
Transconjunctival Blepharoplasty
This lower eyelid technique is ideal for patients with only puffy fat pockets or deep tear troughs underneath the eyes. Candidates for transconjunctival eyelid surgery do not need excess skin removed or muscles tightened. They are typically younger patients with a genetic disposition for undereye bags and good skin elasticity. Older patients can also undergo transconjunctival lower blepharoplasty and may benefit from a combination peel or laser resurfacing of the lower eyelids to address mild skin laxity or crepey skin.

How Is Eyelid Surgery Performed?

Your blepharoplasty consultation is an important step in the surgical process. During your meeting with Dr. Lattyak, he will listen to your goals and concerns for the procedure, examine your eyes (including skin quality), and discuss your options.
Dr. Lattyak will also review your medical history to ensure you do not have any medical conditions that could interfere with the success and safety of the surgery or its recovery period. The Eyelid Surgery Procedure
Blepharoplasty usually takes one to three hours, depending on the extent of the surgery. If you have all four eyelids done, Dr. Lattyak will probably work on the upper lids first, then the lower ones.
In a typical blepharoplasty procedure, Dr. Lattyak makes incisions following the natural lines of your eyelids: in the creases of your upper lids and just below the lashes in the lower lids. The incisions may extend into the crow’s feet or laugh lines at the outer corners of your eyes. Working through these incisions, Dr. Lattyak separates the skin from underlying fatty tissue and muscle, removes excess fat, and often trims sagging skin and muscle. The incisions are then closed with very fine sutures.
If you have a pocket of fat beneath your lower eyelids but don’t need any skin removed, Dr. Lattyak may perform a transconjunctival blepharoplasty. In this procedure, the incision is made inside your lower eyelid, leaving no visible scar. In patients with mild skin laxity or crepey skin, Dr. Lattyak may finish by performing a peel or laser resurfacing of the eyelids.
What Is Eyelid Surgery Recovery Like?
Immediately following your eyelid surgery, your eyes may be swollen and bruised, and you will need someone to drive you home. While you may have some discomfort initially, blepharoplasty is a relatively painless procedure. Any persistent pain can be managed with over-the-counter medications. Dr. Lattyak will provide you with detailed aftercare instructions relevant to your surgical plan, so follow these closely to ensure a safe recovery.
After two or three days of resting your eyes, you should be able to read or watch television. However, you won’t be able to wear contact lenses for about two weeks, and even then, they may feel uncomfortable for a while.
Most people feel ready to go out in public (and back to work) in a week to 10 days. By then, depending on your healing rate and Dr. Lattyak’s instructions, you’ll probably be able to wear makeup to hide the remaining bruising. You may be sensitive to sunlight, wind, and other irritants for several weeks, so wear sunglasses and a special sunblock for eyelids when you go out.
Dr. Lattyak will probably tell you to limit your activities for the first three to five days post-op and to avoid more strenuous activities for about three weeks. It’s essential to avoid activities that raise your blood pressure, including bending, lifting, and rigorous sports. You may also be told to avoid alcohol since it causes fluid retention.
When Will I See My Eyelid Surgery Results?
Most patients can see the initial results of their eyelid surgery once the swelling around the eyes has subsided, generally about seven to ten days after surgery. You will notice that your eyelids are smoother and tightened, and your eyes may look more “awake.”
However, healing is gradual, and your scars may remain slightly pink for six months or more after surgery. Eventually, they’ll fade to a thin, nearly invisible white line. On the other hand, the positive results of your eyelid surgery–the more alert and youthful look–will last for years. For many people, these results are permanent.
